Patient Advocacy - Senior Care Advocates
At Bayshore Home Health we provide patient advocates to give peace of mind to family members who need assistance in reviewing technical medical data, making educated decisions regarding long-tem care options, and can often stand-in place of a family member who lives out of the area. We provide skilled nurses to be eyes and ears for our patients and clients who may need assistance juggling the healthcare system and determining what support services may be in their best interest. The clinical expertise of our nurses provides peace of mind to our clients and their family members as we consult on key factors in senior care. Our nurses can work in conjunction or independent of our staff of Geriatric Care Managers depending on the clients needs.
- Accompany client to medical appointments
- Uphold compliance of care plans and medications; reducing hospitalizations
- Provide seamless transitions between hospitalizations to a alternate care settings
- On-going communication between family and health, legal, and financial representatives
- Consult with family and medical professionals
- Expertise in reviewing medical charts and conferring with client’s physician
- Referrals to quality senior support services
